THE YEAR OF THE PH SUPERHERO'S AWARENESS AWARDS 2018
In 2018 PHNA launched our new theme for 5th May World PH Day through to November PH Awareness Month. We are excited to announce our "Year of the Superhero's Awareness Awards" for our Australian PH Community.
There are three categories you can nominate from, choose one or all three.
1. Person living with pulmonary hypertension.
Awarded to the PH patient who exemplifies dedication to the PH community through any combination of the following: raising awareness, participation in advocacy efforts, fundraising, service to PHNA and helping to provide the public with a better understanding of the disease by becoming an advocate for the PH community.
2. Person caring for someone living with pulmonary hypertension.
The Caregiver award is given to a non-patient who not only focuses their efforts on the needs of a loved one with PH and does their best to ensure the patient has the best quality of life possible, but also exemplifies dedication to the PH community through any combination of the following: raising awareness, participation in advocacy efforts, fundraising, service to PHNA and helping provide the public with a better understanding of the disease by becoming an advocate for the PH community in Australia.
3. Allied Health Professional/Outstanding Physician and or Organisation supporting people living with pulmonary hypertension.
The outstanding physician award recognizes an outstanding physician who has been notable in his or her service to PHNA and in promoting excellent clinical care, research, education and advocacy on behalf of PH Patients in Australia.
The allied healthcare professional (includes nurse, physician assistant, technician, therapist, other organisation etc), for contributions to the PH community through any combination of the following: raising awareness, participation in advocacy efforts, fundraising, service to PHNA, and providing kind and compassionate care to PH patients in Australia.
The three winners will be chosen by a judging panel and the winners will be announced and presented with their awards during November Awareness Month 2018. The finalists will receive a certificate of nomination from PHNA. The winners will receive a beautiful boxed award in recognition of the category they have won.
Nominations open on Saturday 5th May and close on 30 October 2018 - Winners announced Nov 30th

(Many thanks to PHA USA who is hosting their own Outstanding Member Awards this year as part of their International conference, from which we adapted our category criteria).
